I, the person named and signed on this form, agree to join the wine club in which I have selected on this form. I agree and accept all the terms and conditions of the wine club. I also understand that all the terms and conditions will remain in effect until I have fulfilled and paid my minimum shipment requirement based on the wine club chosen.

I agree to have Vezer Family Vineyard, LLC (herein referred to as VEZER) charge my credit card for all of my club wine, tasting room purchases, shipping charges, taxes and for any other purchases I have charged to my account. I agree to notify Vezer of any changes or updates to the credit card residing on file.

I testify that I am 21 years of age, and that there is someone over the age of 21 to receive my wine shipments at the ship-to address provided.

I understand that Vezer reserves the right to change, modify, revise, add or delete the benefits, privileges, terms, conditions, and prices, and/or terminate any club member or wine club at any time upon written notice. Also, I understand that this agreement survives any transfer of winery ownership or management.

I understand and agree that the monthly Chieftain VIP Club is a minimum 24 shipment commitment. And that I will be charged whenever stipulated by the club choice I have selected. I understand that the membership can only be canceled after I have fulfilled my minimum membership term and paid in full for all required shipments and wine during that time.

I understand that if I intend to cancel my membership upon the fulfillment of my original shipment requirement, I must notify VEZER in writing by certified mail to VEZER at 2526 Mankas Corner Road, Fairfield, CA 94534, and this notice must be sent at least 30 days before the end of my original club commitment. If I do not follow the above cancellation procedure, the membership will automatically continue.

I understand that the wine shipments can only be sent to states that VEZER is legally allowed to ship.

By providing my signature on this sign-up form I am stating that I am in complete agreement with all of the terms of my membership and approve VEZER to bill my credit card throughout the duration of my membership.
